Patients with depression, particularly treatment-resistant depression, could be referred to the rTMS clinic.

Adults with suspected early-onset dementia, or other neurocognitive disorders, could be referred to the memory clinic for assessment, diagnostic clarification or treatment recommendations.

What is the referral process?

For rTMS eligibility assessment and treatment, family doctors, nurse practitioners, and psychiatrists can fax a letter outlining their patient's current medical history (including past treatment trials), current medications and dosages, and complete past medical history to BrainWave Clinic (fax: 204-417-5266).

For memory consults, medical doctors (specialists or primary care providers), physician or clinical assistants, and nurse practitioners can fax a letter outlining their patient’s relevant medical history and the reason for referral to BrainWave Clinic (fax: 204-417-5266).

The optional referral forms below can be used if convenient. For Accuro users, a fillable version of these forms can also be downloaded via Form Editor > Tools > Publish / Download > Search "BrainWave" > select the most updated form desired (published in 2024) > Download.
